USS Liberty Incident (Wiki)

June 1967 Isreal was preparing to seize the Golan Heights from Syria.
The Wiki page says the Liberty (Wiki) was a "Technical Research Ship" which is government speak for a spy ship (Wiki).

USS Pueblo (Wiki)

January 23, 1968 was attacked by North Korean naval vessels and MiG jets.  The Pueblo was an ELINT/SIGINT ship.

April 17, 1975 - Pol Pot banned by decree the existence of ethnic Chinese, Vietnamese, Muslim Cham, and 20 other minorities.

Iran-Iraq War (Wiki)

Iraq launched a full-scale invasion of Iran on 22 September 1980.  The U.S. supplied Iraq with several billion dollars worth of economic aid, the sale of dual-use technology, non-U.S. origin weaponry, military intelligence, Special Operations training, and direct involvement in warfare against Iran

1986 - 1989 - The Anfal campaign included the use of ground offensives, aerial bombing, systematic destruction of settlements, mass deportation, firing squads, and chemical warfare, which earned al-Majid the nickname of "Chemical Ali".  Maybe 100,000 dead.

August 20, 1998 U.S. military attacks Osama bin Laden using cruse missiles aginst the Al-Shifa pharmaceutical factory (Wiki) in Sudan.  There were no WMDs there.

5 Feb 2003 - UN Security Council hears Sec. of State Powell's case to go to war with Iraq.
20 March to 1 May 2003 "Shock and Awe" invasion of Iraq (Wiki).
The damage did not end with the end of war but continuted on when the Iraq military was fired leaving the country to be looted.
In the book "Why Leaders Lie: The Truth About Lying in International Politics" by  John J. Mearsheimer he explains why leaders lie to their own population but not to other countries.
